NOTE: This form can only be used to extend the end date on your work authorization between academic semesters. It cannot be used to extend CPT beyond your degree completion date or to change employers. You must submit this form at least 8 business days before the end date on your work authorization.

Note: Work Term Evaluation must be submitted before Co-op Extension can be processed.
